Structure of the 11-Column Personal Register:
Current Number: Serial number of the paper/file.
Date of Receipt: Date the paper is received.
From Whom Received: Designation and address of the sender.
Letter Number & Date: Original reference number and date of the paper.
Subject: Brief description of the subject matter.
Endorsement/Reference: Details of how the paper is disposed of or referenced.
Officer to whom sent: To whom the paper was submitted/forwarded.
Date of Disposal: The date the file was closed/sent out.
Final Disposal File Number: Reference for archived files.
Remarks: Any additional notes, such as interim replies or delays.
Reminder Column: Tracks when reminders were sent to ensure follow-up.
Key Guidelines:
Maintenance: Entries are made on the day of receipt.
Checking: Superintendents and higher officers check the register regularly to ensure files are not unduly delayed.
Purpose: It is essential for monitoring and ensuring accountability in administrative work in AP.
